General Comment: #27.Observed arm rest on chair in room 312 and 105 worn/damaged. Repair arm rest on chair or replace chair.**Furniture, fixtures, carpets and other accessories shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).#26. Observed nail popping out(nail popper) along upper section of walls throughout in different rooms.
