4-601.11(C )/4-602.13 Keep the equipment clean to avoid build up of dust, food residue or debris. Observed spilled food on table holding slicer. Slicer had been cleaned and covered with a plastic bag. Inside of plastic bag was oily with some sort of greasy material. CDI - PIC discarded bag, recleaned and sanitized slicer and table, and covered again. He will also move slicer and buffalo chopper away from soiled drainboard side of 3 comp sink. |

Observed two holes in cinder block wall to left of dish machine. Observed corner of baseboard tile on wall between kitchen and dish room had been chipped off and was missing.

Observed dusty buildup on vent covers on vent above 3 comp sink. Observed dusty buildup on fan in dish room which was aimed at clean drainboard side of dish machine. Observed floor in corner between washer and dryer in need of much better cleaning. |
