Observed a scoop being held in a container of sautéed onions in prep flip top cooler in which handle of scoop was resting on onions. CDI: Employee repositioned scoop handle to be above food. REPEAT. Points not escalated due to multiple violations on previous report. |

6-501.11 Floors, walls, and ceilings including the attachments such as soap and towel dispensers; light fixtures; and heat/ac vents shall be maintained in good repair. Observed ceramic tile that came off of molding inside walk in cooler next to door on left side. PIC stated a service request has been submitted for repair. Also observed grout worn away in between floor tiles underneath dish machine. Replace missing grout. |
