2-402.11 Effectiveness - Hair Restraints (C)-Except for when working with packaged food or at register, PIC must wear an effective hair restraint since they are moving from cookline to register and work with open food during the food prep process. Upon entry PIC was not wearing a hair restraint; however found a hat without being prompted by EHS. |

6-501.11 Repairing - Premises, Structures, Attachments, and Fixtures - Methods (C)-Wall damage noted behind ice bin. Replace cracked tile at entryway to hall that leads to dry storage room. Replace water damaged ceiling tile at stairs. Physical facilities shall be maintained in good repair.
6-501.12 Cleaning, Frequency and Restrictions (C) Clean carpet in hallway in the high traffic areas. Physical facilities shall be cleaned as often as necessary to keep them clean. |
