Surfaces should be smooth, sealed and easily cleanable. Repair areas as needed. Observed cracked flooring in the facility. The damaged flooring must be properly repaired, replaced, or effectively sealed to ensure a smooth, cleanable, and non-absorbent surface. Additionally, peeling paint was observed on the ceiling above the stove and the three-compartment sink area. This surface requires complete repair. All loose and peeling paint must be fully removed prior to repainting. Painting over peeling paint does not correct the underlying issue, as evidenced by continued paint failure in this area. The ceiling must be properly prepared, repaired, and refinished to prevent further deterioration. NO POINTS TAKEN
