4-602.11: Observed in the ice machine is an accumulation of residue along the walls of the machine. Ensure these areas are cleaned and sanitized and then cleaned and sanitized at a frequency necessary to maintain them clean. The PIC instructed an employee to clean and sanitize the ice machine during the inspection. This surface was observed clean by the end of the inspection and no follow-up is being issued. This is a repeat violation from the June 22, 022, March 9, 2023, June 27, 2023, and the March 26, 2024 inspections. A compliance conference has already been held regarding this item. Facility has a new cleaning procedure for the ice machine and detailed instruction was provided to the employee responsible for its cleaning during today’s inspection. 4-602.11: (D) FOOD-CONTACT SURFACES shall be cleaned less than every 4 hours if in storage of food within proper temperatures, food is prepared in refrigerated room, if practice has regulatory approval, or UTENSILS stored in 135F water. (E) Surfaces of UTENSILS and EQUIPMENT contacting food shall be cleaned any time contamination occurs, at least 24 hours for certain UTENSILS (i.e. iced tea dispensers), before restocking self-service EQUIPMENT, in EQUIPMENT such as ice bins in frequency to prevent mold. At a frequency to preclude accumulation of soil or mold.

Non-food contact surfaces clean
4-601.11: The following locations are observed to have an accumulation of food debris/soils that are in need of being cleaned: -Shelving in the walk-in cooler -Cages over the fans in the walk-in cooler Ensure these areas are cleaned and then are included in the regular cleaning schedule to be cleaned at a frequency necessary to maintain them clean. This is a repeat violation from the 6/27/2023 and 3/26/2024 inspections and the facility will be receiving a Notice to Correct documenting these violations. 4-601.11: (B) The FOOD-CONTACT SURFACES of cooking EQUIPMENT and pans shall be kept free of encrusted grease deposits and other soil accumulations. (C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of dust, dirt, FOOD residue, and other debris.

6-101.11: The floor of the walk-in freezer is observed to have a visible gap where the floor has lifted. Ensure repairs are made to the freezer floor to make it smooth, nonabsorbent, and easily cleanable. This is a repeat violation from the June 27, 2023 and March 26, 2024 inspections and the facility will be receiving a Notice to Correct documenting the violation. 6-101.11: (A) Materials indoor shall be SMOOTH, durable, and EASILY CLEANABLE. Areas subject to moisture must be Nonabsorbent. (B) In TEMPORARY FOOD ESTABLISHMENT, floor may be material effectively treated to control dust and mud. Walls and ceilings may be material that protects interior from weather and windblown dust and debris.

6-101.11: The main kitchen hand sink is observed to have caulking that is no longer smooth, nonabsorbent, and easily cleanable. Ensure the current caulking is removed and a fresh seal is placed to create a smooth, nonabsorbent, and easily cleanable surface. This is a repeat violation from the 3/26/2024 inspection Adam the facility will receive a Warning Notice documenting the violation. 6-101.11: (A) Materials indoor shall be SMOOTH, durable, and EASILY CLEANABLE. 6-501.12: The floor in the dry storage closet containing food is observed to have an accumulation of food debris/soils. Ensure this floor is cleaned to remove the current accumulation of debris/soils band then cleaned at a frequency necessary to maintain them clean. 6-501.12: (A) PHYSICAL FACILITIES shall be cleaned as often as necessary to keep them clean. (B) Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of FOOD is exposed such as after closing.

6-501.12: The floors under the kitchen equipment are observed to have a heavy accumulation of food soils/debris. Facility performs routine deep cleaning of these areas during times of slow business when they can pull all the equipment away from the walls. Ensure these areas are cleaned to remove the current accumulation of food soils/debris and then cleaned at a frequency necessary to maintain them clean. 6-501.12: (A) PHYSICAL FACILITIES shall be cleaned as often as necessary to keep them clean. (B) Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of FOOD is exposed such as after closing.

6-501.11: There is observed a large amount of ice accumulation in the walk-in freezer along the walls of the left internal wall and along the back wall/under the freezer unit. Ensure the ice accumulation is removed and that the unit is repaired to prevent the ice accumulation. 6-501.11: PHYSICAL FACILITIES shall be maintained in good repair.

Adequate ventilation and lighting; designated areas used
6-303.11: There is observed a lightbulb out in the walk-in cooler and the walk-in freezer. Ensure these lightbulbs are replaced with functioning ones in order for there to be sufficient light in these areas. This is a repeat violation from the June 27, 2023 and the March 26, 2024 inspections and the facility will be receiving a Notice to Correct documenting the violation. 6-303.11: The light intensity shall be at least a certain lux and present in certain areas.

All food employees have food handler training
410 ILCS 625 and IAC Section 750.3430 (C): Facility was unable to verify all employees’ food handler certifications. Ensure that all employees complete an approved food handler course and receive a food handling certificate. Ensure these certificates are able to be verified when requested during an inspection. Employees have 30 days from the date of hire to obtain this certification. 410 ILCS 625 and IAC Section 750.3430 (C): Facility is not in compliance with the food handler law. All food handlers, other than someone who has the food service sanitation manager certificate, shall receive or obtain training in basic food handling principles as outlined in Subpart M of the Illinois Food Code. Food handlers working at a temporary food establishment are exempt from this requirement.

Allergen training as required
410 ILCS 625/3.07 Sec. 3.07: PIC does not have their Allergen training. Ensure that the CFM acting as the PIC has obtained their Allergen Training from an approved provider and is able to provide evidence of completion with a certificate upon request during the inspection. 410 ILCS 625/3.07 Sec. 3.07: Allergen awareness training. (b) Unless otherwise provided, all certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days after employment and every 3 years thereafter.
